Medical paperwork is needed BEFORE the start of school and will be accepted starting August 5. These forms can all be found on the Pius Website under the “Health” page. All medications, including over the counter medications, need to come from the Nurse’s office during the school day. Email holly-steinbach@cdolinc.net with any questions.
If your child:
- has or needs an inhaler or an EPI pen, an Asthma/Allergy Action plan signed by your doctor dated after May 1, 2026 is required.
- wants to self-carry their inhaler and/or epi pen, a Self- Management form with their action plan is required.
- wants to keep their inhaler and/or EPI pen in the health office, the Pius RX permission form and the action plan are required. This will be updated for next school year after May 23 on the website.
- has diabetes, seizures, or any other chronic health conditions that require an action plan, these will need to be turned in and dated after May 1, 2026.
- requires ANY prescription medications during the school day (even for a few days during the school year), an RX permission form must be on file in the health office, and the medication MUST come in the original bottle/container.
Please ensure that you have filled out all the medical forms on PowerSchool as these give permission for me to give over the counter medications and provide other vital information.
If you have not gone to a Lincoln Diocese School for elementary school (out of state transfer, coming from public school/home school, or another school) be sure to bring an updated copy of immunizations.
