All student-athletes are required to have a sports physical completed between May 1, 2025, and the start of the sports or activity the students is trying out for or participating in. There is no standard form required; medical offices will have a standard form they use, and that will suffice.

Each student-athlete also needs to complete our Wellness Packet forms, found on our Athletics home page.

Completed forms should be emailed to activitiesoffice@piusx.net brought directly to the main school office anytime now or throughout the summer. No one is allowed to try-out or participate without turning in the completed form.

We encourage all families to schedule these appointments soon to avoid the late rush of appointments right before the fall seasons begin, and to turn in copies of the completed form soon thereafter to avoid them getting misplaced.
