This form is only to request items, not order or pay for uniform items. When we receive your request, we will verify your requested items are in stock in our school uniform office, gather them, and tell you the total cost. You can pay at that time, and we’ll send the items home with your Pius X student, unless other arrangements are made. Thank you.
